Terms and Limitations
This is a dental savings plan NOT dental insurance. It cannot be combined with ANY dental insurance. It is good at Modern Dental Concepts only. Therefore, if you are referred to a specialist, they will NOT offer this discount. Treatment for dental injuries covered by workmen’s comp, disability insurance, lawsuit, or outside medical care are not covered under this plan. This plan is NON-Transferable: Family members cannot be substituted in for another family member. This plan is NON-Refundable: no refunds will be given if the patient chooses not to use their dental plan. Member rates are subject to change annually. Retail Fees subject to change at any time. This offer cannot be combined with any other offer. Plan terms may be subject to change.

- Payments are due at time of scheduled appointment.
- If you choose to use Care Credit for payment there will be no discount given for that treatment due to merchant fees.
- Cannot be used for orthodontic, implant, or facial aesthetic treatment.
Once membership is paid, the member will be entitled to the discounted fee for dental services for up to 12 months. Members, spouses, dependent children under the age of 19 or full-time students up to 23 years of age are eligible. Members have the right to cancel at any time by submitting a written request to the dental practice.
