Fees & Payment Policy
Our primary mission is to deliver the best and most comprehensive dental care available. In an effort to keep dental costs down while maintaining a high level of professional care, we have established the following payment plans for the use of our patients.
Fees may be paid as follows:
*Mastercard and Visa are accepted – due to rising credit card fees, there will be a $2 up-charge applied to all transactions under $25

If you have insurance, we will help you to determine the coverage you have available. We ask that you assign your insurance benefit to us. The balance will be arranged for you to pay as listed above.
Professional care is provided to you, our patient, and not an insurance company. Thus, the insurance company is responsible to the patient and the patient is responsible to the office. We will help in every way we can in filing your claim and handling insurance questions from our office on your behalf.
Please note: Modern Dental Concepts requires payment prior to the beginning of your treatment. If you choose to discontinue care before treatment is complete, your refund will be determined upon review of your case. For plans requiring more than 2 appointments, alternative payment arrangements may be provided. For patients with dental insurance we are happy to work with your carrier to maximize your benefit and directly bill them for reimbursement for your treatment.
A fee of $50 may be charged for every half hour of appointment missed without a 48 hour prior notice. Patients who miss or cancel more than 2 times in a calendar year without 48 hour notice may be subject to dismissal.
$30 fee for returned checks.
An 18% finance charge will accumulate for any unpaid balances that are over 60 days at the office.
